13-Week Contract Job Overview A healthcare organization in Massachusetts is hiring an experienced RN Hospice Case Manager for a travel assignment in the South Dennis, MA. This role involves providing hospice nursing care and case management services to patients in homes and healthcare facilities. Responsibilities Provide hospice nursing care and case management Complete patient assessments and monitor condition changes Develop and follow patient care plans Coordinate care with physicians and hospice team members Educate and support patients and families Complete admissions, charting, and documentation on time Supervise Hospice Aides as needed Maintain compliance with hospice and nursing standards Requirements Active Massachusetts RN License Minimum 2 years of Hospice RN Case Management experience Hospice Home Care experience required Strong communication and documentation skills Ability to work independently Preferred IV Certification BSN For more details contact at or Call / Text at 516-862-1169.
