Full-Time Rate Per Visit:
$60 per visit + 20 cents per mile. If it's more than 30 miles, then only you'll get that 20 cents per mile. Qualifications Must be a fully licensed Registered Nurse by the Massachusetts Board of Registration in Nursing. Must have at least two (2) years of recent experience in the direct care of elders or adults with disabilities. Responsibilities Complete and coordinate all applicable clinical assessments and evaluations. Develop and review each member's AFC Plan of Care on an ongoing basis. Provide oversight to AFC caregivers in conjunction with the Care Manager. Review the PCP Summary Form. Monitor the health status of all members to ensure appropriate AFC services are delivered. Report changes in a member's health status to the member's Primary Care Provider (PCP). Review AFC caregiver logs at least every 30 days. Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6). Complete nursing progress notes following each on-site visit or encounter, or more frequently if the member's condition requires. Submit semiannual health status reports to the member's PCP. Plan and implement discharges and transitions from the AFC program. Conduct orientation for each AFC caregiver before personal care begins. Provide ongoing education and training to AFC caregivers regarding health and aging.

$43.77 - $52.71 per hour
