Monday-Friday, 8:30 AM•5:00 PM (No weekends or holidays)
Duration:
13-week contract to start, with strong potential for extension
Pay Rate:
$55/hour Position Overview The RN Case Manager plays a key role in coordinating comprehensive care for patients with substance use disorders. This position focuses on developing individualized care plans that address each patient's medical, psychological, and social needs. Working closely with a multidisciplinary team, the RN Case Manager ensures patients have access to necessary resources and services to support recovery and improve health outcomes. This role also emphasizes patient education, engagement, and long-term care coordination. Key Responsibilities Identify and engage patients eligible for RN case management based on diagnosis, treatment plans, risk factors, compliance, ER utilization, or program criteria Collaborate with PCPs, RNs, pharmacists, and other providers to develop and update patient care plans Coordinate referrals and services across internal departments and external agencies (e.g., Social Services, Behavioral Health, WIC, Dental, Vision, Pharmacy) Ensure patients are scheduled for follow-ups, labs, and necessary appointments; conduct outreach for missed visits Document all patient interactions and interventions accurately in the EHR/EPM system Support patient education, self-management, and adherence to care plans
Qualifications Required:
Active Massachusetts RN license Experience in community health or nurse case management preferred Strong communication skills and ability to work independently Reliable attendance and a positive, team-oriented attitude
Preferred:
Certification in Case Management (CCM) or related specialty Experience working with patients in substance use treatment programs Familiarity with community resources supporting individuals with substance use disorders Bilingual (Spanish, Khmer, or Russian) Ideal Candidate Profile We are seeking a self-starter who is passionate about patient care and care coordination.
