  • What You'll Do
  • + Manage and coordinate a
  • dedicated caseload of home health patients
  • + Perform comprehensive assessments and develop individualized care plans + Provide
  • 1:1 patient care
  • in the home setting + Collaborate with physicians, therapists, and interdisciplinary team members + Adjust care plans based on patient progress and clinical findings + Build meaningful relationships with patients and families

Physical Requirements:
  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
