- Home Health Case Manager

- What You'll Do
- + Manage and coordinate a
- dedicated caseload of home health patients
- + Perform comprehensive assessments and develop individualized care plans + Provide
- 1:1 patient care
- in the home setting + Collaborate with physicians, therapists, and interdisciplinary team members + Adjust care plans based on patient progress and clinical findings + Build meaningful relationships with patients and families

Physical Requirements:
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
