Job Description
The Home Health Wound Care Certified Registered Nurse (RN) is responsible for providing specialized wound assessment, treatment, and education to patients in the home setting. This position focuses on the prevention, management, and treatment of acute and chronic wounds while collaborating with physicians, clinicians, patients, and caregivers to promote optimal healing outcomes and prevent complications. The Wound Care RN ensures compliance with all agency policies, physician orders, regulatory requirements, and evidence-based wound care practices.
QUALIFICATIONS
Current unrestricted Registered Nurse (RN) license in the Commonwealth of Massachusetts. Current Wound Care Certification preferred or required depending on agency needs (examples include WCC, CWCN, CWON, or comparable certification). Minimum of one (1) year of wound care experience preferred. Home health experience preferred. Current CPR certification required. Strong assessment, clinical decision-making, and organizational skills. Ability to work independently and manage time effectively in a community-based setting. Proficient computer and electronic medical record documentation skills. Valid driver's license, reliable transportation, and ability to travel to patient homes.
