Choose your preferred locations from below: 1) Newton, Wellesley, Needham, Brighton, Allston, Brookline, Chestnut Hill or 2) Framingham, Natick, Hopkinton, Holliston, Ashland 3) Boston, Dorchester, Jamaica Plain, Roslindale, Mattapan, Roxbury, Hyde Park. This is a per diem position with flexible hours, visit work schedules and an excellent reimbursement package. Our nurses can choose their preferred geographical area to do home care visits. Provides initial and ongoing assessments to identify the client's medical needs, level of functioning, and ability to communicate and learn, including OASIS assessments at appropriate time points when applicable. Obtains orders and makes referrals to other disciplines when appropriate. Regularly re-assesses the client's nursing needs. Initiates the plan of care and necessary revisions. Furnishes those services requiring substantial and specialized nursing skills. Coordinates services, and informs the physician and other personnel of changes in the client's condition and needs. Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure. Per diem position, Preferred Locations, Flexible Schedule Pay range up to $110 per visit depending on type of the visit Required Qualifications/Licenses

• Current license as a Registered Nurse in the Commonwealth of Massachusetts required
• Current CPR required

Additional Requirements:

• Prior experience in home health care preferred
• Valid state driver's license
• Must have a reliable car with insurance coverage.
