Your Role:
Provide direct patient care and case management in the home Conduct patient admissions and develop individualized care plans Obtain medical history and perform brief physical exams (vitals, mental status, diet, etc.) Deliver skilled nursing care in compliance with HIPAA Evaluate effectiveness of nursing services and coordinate total patient care Submit patient notes and documentation in a timely manner
Qualifications:
Licensed LPN in Massachusetts Physical exam within 12 months prior to first patient contact TB test within 12 months & CPR certification Valid driver's license Ability to attend 2-day orientation at Taunton office Make a Difference in Home Healthcare Join a team that values your skills and empowers patient independence.
