The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while managing a diverse patient population. Responsibilities Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop, implement, and evaluate individualized care plans based on patient assessments and goals. Administer medications and treatments as prescribed, including pain management techniques. Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services. Educate patients and their families on health management, disease prevention, and self-care techniques. Provide support for patients with disabilities, ensuring they receive appropriate care tailored to their needs. Participate in critical care transport when necessary, ensuring safe transitions for patients requiring specialized attention. Qualifications Current Registered Nurse (RN) license in the state of MA. 1 year of Certified Home Care and case management experience. Strong knowledge of anatomy and physiology to inform patient assessments and interventions. Excellent interpersonal skills to effectively communicate with patients, families, and healthcare teams.
