Overview of Job Description:
The Home Care Director of Nursing (DON) is a Register Nurse leader who will play a pivotal role in shaping the delivery of nursing services. The Homecare DON will be responsible for overseeing and coordinating the clinical operations of homecare programs and ensuring the highest standards of care are established and maintained. This is a leadership position that demands a deep understanding of nursing practices, regulatory compliance, and a passion for improving the lives of homecare patients.

- 5:00 PM
- This can be a hybrid role and may require homecare visits, and/or on-the-road training as needed.
- Will hold current RN license in Massachusetts.
Preferred BA or BSN and minimum 2 years Homecare experience.
Key Responsibilities:
Clinical Leadership:
Provide clinical leadership and direction to the nursing team, ensuring that care is delivered in accordance with established standards and protocols on an ongoing and day-to-day basis.
Quality Assurance:
Lead, develop, and implement clinical related Quality Assurance and Performance Improvement initiatives (QAPI) to enhance patient outcomes and satisfaction and identify clinical risk.
Regulatory Compliance:
Stay abreast of federal, state, and local regulations and ensure that our services adhere to all relevant compliance standards including but not limited to CHAP, Dept Public Health (DPH), and CMS Conditions of Participation (CoP) standards. Oasis submissions/IQES audits routinely minimum 1x wk.
Team Management:
Clinical oversight and leadership of nursing and HHA staff. Recruit, hire, onboard, train, and manage a clinical team of nurses and caregivers, fostering a culture of collaboration, accountability, and professional growth. Collaborate with HR and Non-Certified leadership as indicated to conduct performance evaluations of clinical and HHA staff.
Patient Care Coordination:
Oversee the coordination of patient care, including assessments, development of care plans, and the evaluation of patient progress. Coordinate care with physicians, families, and other stakeholders like PT, SW etc.
Budget Management:
Manage the budget for the nursing department, optimizing resource allocation while maintaining quality care.
Collaboration:
Collaborate with other Interdisciplinary Departments (IDT), healthcare providers, and community organizations to ensure seamless and holistic care for our patients.
Performance Metrics:
Establish and monitor key performance indicators (KPI) to track the effectiveness of nursing services and identify areas for improvement ie (ROCKS)
