We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Norwood. In this role, you will provide comprehensive care to patients in their homes, ensuring the highest standards of nursing practice are met. You will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records.Key Responsibilities:
Conduct thorough assessments of patients' health conditions in their home environments. Develop personalized care plans and collaborate with multidisciplinary teams to deliver optimal patient care. Administer medications and treatments as prescribed by the physician. Provide education and support to patients and their families on disease prevention and health maintenance. Monitor patient progress and modify care plans as necessary to achieve health goals. Maintain accurate, detailed reports and records in accordance with regulatory requirements. Ensure compliance with all healthcare regulations and quality standards.Qualifications:
Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation. One (1) year experience as a RN. Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.Additional Information:
