Job Overview Join our dedicated healthcare team as a Home Health Registered Nurse (RN), where your expertise will directly impact patients' lives by delivering compassionate, high-quality care in their own homes. This dynamic role offers the opportunity to utilize a broad range of clinical skills, including IV infusion, wound care, medication administration, and patient assessment, all while promoting independence and well-being. As a vital member of our home health team, you'll coordinate care plans, educate patients and families, and ensure seamless communication with interdisciplinary teams. Duties Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans. Administer medications, including injections and IV therapies, following strict aseptic techniques and safety protocols. Perform wound care procedures such as suturing assistance, ostomy management, and dressing changes. Monitor vital signs, manage acute pain, and provide interventions for complex conditions like dialysis or cardiac issues. Educate patients and caregivers on disease management, medication adherence, nutrition, and safety measures at home. Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner while maintaining HIPAA compliance. Coordinate with physicians and interdisciplinary teams for ongoing case management, discharge planning, and follow-up care. Requirements Valid RN license in the state of practice with current BLS (Basic Life Support) certification. Proven experience in diverse settings such as hospital medicine, ICU, emergency room, or medical-surgical units. Strong clinical skills including IV insertion, phlebotomy, catheterization, and aseptic technique. Familiarity with EMR/EHR systems such as Epic or Cerner for accurate documentation. Experience working with individuals with disabilities or in long-term care environments like nursing homes or assisted living facilities. Knowledge of ICD coding (ICD-9/10), CPT coding, infection control practices, and trauma medicine standards. Ability to perform physical examinations and diagnostic evaluations in a home setting; comfort with laboratory specimen collection is a plus. Excellent communication skills for triage assessments and patient education; critical thinking to handle urgent situations effectively.
