!! Hands and Hearts Hospice is a locally owned and operated Hospice company based in Wellesley, MA that services patients inside the 495 beltway. Responsibilities:
  • Provide compassionate and comprehensive nursing care to patients in a hospice setting
  • Conduct initial assessments and develop individualized care plans
  • Administer medications and treatments as prescribed
  • Monitor and document patient progress, symptoms, and vital signs
  • Collaborate with interdisciplinary team members to coordinate patient care
  • Educate patients and their families on disease management, symptom control, and end-of-life care options
  • Provide emotional support and comfort to patients and their families
  • Ensure compliance with infection control protocols
Qualifications:
  • Valid RN license in the state of Massachusetts
  • Minimum of [1] years of nursing experience, preferably in hospice or palliative care
  • Experience working with pediatric patients is highly desirable
  • Proficiency in triage, tube feeding, laboratory procedures, medication administration, phlebotomy, and acute care
  • Familiarity with electronic health record systems (e.g., Athenahealth) is a plus
  • Strong communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment
  • Compassionate and empathetic approach to patient care
