Location :
Across Cape Cod Compensation :
$48.00-$50.40 per hour (based on experience)
Schedule :
Full-time with rotating on-call responsibilities
Key Responsibilities :
Monitor and respond to the daily health needs of residents Coordinate and schedule preventive, specialized, and emergency healthcare appointments Oversee medication management, including ordering, administration, and documentation per DPH, MAPS, and agency policies Monitor medical treatment to ensure follow-through by program staff Act as a liaison between families, referring agencies, and community resources regarding medical needs Organize and participate in psychiatric consults and medication reviews Maintain accurate resident medical records and required program reports Obtain MAP certification as a Train-the-Trainer and ensure staff are properly trained and supported Provide in-service training and medical education, including health and human sexuality topics Participate in intake and discharge planning, including hospital discharge coordination Maintain adequate medical supply inventory Assist in program development, implementation, and evaluation Provide direct care support as needed Rotate on-call responsibilities for one week at a time
Requirements :
High School Diploma or GED required Associate or Bachelor's degree in nursing Relevant nursing experience required Psychiatric or special needs experience preferred Valid driver's license MA Registered Nursing license
