Our mission is to provide an empowering and dignified hospice experience for every patient we serve. We are partnering with a growing hospice organization serving patients throughout the Greater Springfield region. This organization is known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings while maintaining a supportive, team-oriented clinical environment. We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week). Pay and Schedule $85,000
  • 105,000 annually based on experience Monday-Friday schedule w/ some on-call rotation Flexible scheduling structure with leadership support Why This Role Stands Out Caseload of approximately 14-16 patients Current census of approximately 100 patients Approximately 4-5 visits per day Majority of visits are facility-based, supporting more efficient scheduling Supportive leadership and collaborative clinical team Geographic scheduling whenever possible to help reduce drive time Structured onboarding and mentorship support Responsibilities Manage hospice patient caseload across home and facility settings Perform patient assessments and develop individualized care plans Provide skilled nursing care and symptom management Coordinate with interdisciplinary team members Maintain accurate and timely clinical documentation Maintain documentation using WellSky EMR Support patients and families throughout the hospice journey T erritory Coverage includes Springfield, Holyoke, and Westfield Patients are assigned geographically to support efficiency Mix of facility and home-based hospice care Travel throughout the territory is required Benefits Mileage reimbursement at IRS rate Medical, dental, and vision insurance 401(k) Tablet, hotspot, and cell phone provided Structured onboarding and shadowing support Leadership mentorship and clinical support Requirements Active RN license At least one year of clinical experience (acute care, home health, or hospice preferred) Valid driver's license and reliable transportation Apply Today If you're seeking a hospice RN role with a manageable caseload, scheduling flexibility, and strong team support, we encourage you to apply to learn more.
