Position Summary:
The RN Case Manager is responsible for coordinating and overseeing resident care to ensure the highest quality outcomes, regulatory compliance, and resident satisfaction. The RN Case Manager serves as a liaison between residents, families, physicians, interdisciplinary team members, and outside providers to facilitate comprehensive care planning and effective care transitions. Essential Duties and Responsibilities:
Conduct comprehensive resident assessments and participate in the development, implementation, and evaluation of individualized care plans. Coordinate care among nursing staff, physicians, therapy departments, social services, dietary, and other interdisciplinary team members. Monitor resident progress and clinical outcomes, identifying and addressing changes in condition promptly. Review and manage hospital admissions, readmissions, discharges, and transfers to ensure continuity of care. Participate in care plan meetings with residents and families, providing education and support regarding care needs and goals. Collaborate with physicians and nurse practitioners to ensure timely communication and implementation of orders. Assist with utilization review, clinical documentation, and case management processes to support quality care and reimbursement. Ensure compliance with federal, state, and facility regulations, policies, and procedures. Monitor and track quality measures, identifying opportunities for performance improvement. Facilitate referrals to specialty services, community resources, and outside providers as needed. Advocate for residents' rights, preferences, and overall well-being. Participate in quality assurance and performance improvement (QAPI) activities. Maintain accurate and timely clinical documentation within the electronic medical record. Assist with infection prevention, risk management, and clinical initiatives as assigned. Support discharge planning and coordination of post-discharge services to promote successful transitions. Qualifications:
Current Registered Nurse (RN) license in good standing. Minimum of 2 years of nursing experience; long-term care or skilled nursing experience preferred. Strong knowledge of nursing home regulations, MDS process, and care planning. Excellent communication, organizational, and problem-solving skills. Ability to work collaboratively with interdisciplinary teams. Proficiency with electronic medical records and Microsoft Office applications. Case management or care coordination experience preferred. Physical Requirements:
Ability to stand, walk, bend, and lift as required to perform job duties. Ability to respond quickly in emergency situations. Ability to communicate effectively with residents, families, staff, and healthcare providers. Benefits:
