Job Description

Our company is looking to hire a Home care admissions nurse .
Responsibilities:
Completes an initial home health assessment of patient and family to determine home care needs; provides a full physical assessment and history of current and past illnesses. Utilizes the nursing process in planning, providing, and evaluating skilled needs. Completes a comprehensive patient assessment reflected in accurate OASIS scoring. Develops a care plan with goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes patient and family in planning and revising care.
Required:
Graduate of an accredited school of nursing. Associate degree in nursing. At least 1 year of recent home Care experience Ability to exercise initiative and independent judgment. CPR certification. The pay is $150-$160 per admission.
