Adult Foster Care (AFC) Job Summary We are seeking a compassionate Licensed Practical Nurse (LPN) to join our Adult Foster Care (AFC) team. This community-based role provides clinical oversight, member assessments, caregiver support, and health monitoring for adults and elders living safely at home. This position offers flexibility, autonomy, and meaningful one-on-one member interaction.
