Job Description

M-F 8am - 4pm. No weekends. No holidays. $3k Sign On Bonus. 403b Matching. The Registered Nurse shall administer, coordinate, and monitor all health care services for participants utilizing day services including but not limited to: day habilitation, community-based day supports, employment services, and brain injury day services. In doing so, he/she shall establish and maintain appropriate relationships with families, other health care providers, pharmacists and human service providers.
QUALIFICATIONS
Current Licensure by the Commonwealth of Massachusetts as a Registered Nurse. Education must commensurate with level of Nursing Degree Physical capabilities required to perform direct service duties Possession of a valid driver's license, a safe driving record, and an agreement to use one's personal vehicle for business purposes if so required. One year experience in a community health care setting; experience working with people with developmental disabilities is preferable. Acceptable Criminal Offender Record Information (CORI) and Fingerprint check.
ESSENTIAL DUTIES
Provide oversight nursing to all programs at Cooperative Production that provide day services to participants. This can include traveling to all day sites. Supervise LPN staff that are assigned to work out of the day sites, work with Director and Health Care Services and HR to supervise and manage LPN nurses at each day site. Cover call outs at day service locations or coordinate coverage of call-outs Participate in all pre-admission evaluation studies. Participate in the periodic review/re-evaluation of the type, extent, and quality of services and programming. Ensure that each participant possesses a complete medical case history. Participate in all interdisciplinary team reviews and attend all related meetings as assigned by Director of Health Care Services or designee. Recommend lab tests, x-rays, EEGs and other diagnostic tests as needed. Note test results. Ensure medical records and procedures are in compliance with local, state, and federal regulations. Prepare for and participate in funding surveys including but not limited to CARF and DDS survey and certification. Participate in the referral to appropriate community resources. Coordinate medical services with residential nursing staff when needed. Confer with other health care professionals and team members as needed (i.e., dentists, audiologists, therapists, physical, occupational, speech, program coordinators, etc.) re: participant progress and treatment plan/programming. Communicate participants' health needs and plan of care to Day Habilitation staff & Residential as appropriate. Participate in Quarterly and Annual meetings for participants of day habilitation as needed. Participate in provider meetings, ISP meetings and team meetings as assigned and needed. Control communicable diseases and infections by (a) identification and assessment; (b) reporting to proper authorities; (c) initiation of appropriate protective/preventative measures; (d) documentation. Respond to emergency situations as needed and document properly. Update re-certification orders annually. Document participant immunizations and update as needed.
Provide documentation re:
participant's responses to medical interventions/nursing notes. As applicable, provide participant instruction in such areas as safety, first aid, health and nutrition, personal hygiene, and exercise. Communicate and coordinate all relevant information to the nurses and staff at both the day program and at the participant's residence (all shifts). Act as a resource for the residential program as needed. Participate in training all staff on assigned topics which include but are not limited to: blood born pathogens, diet textures, wound management etc. during new hire orientation and at staff meetings. Become Safety Care Certified and participate in behavior management as needed. Become a back-up MAP trainer and complete MAP training once per year.
