Your Role:

Collect patient medical histories and assess physical health (vitals, mental status, diet, etc.) Educate patients and families on nursing services and care plans Provide skilled nursing care in accordance with care plans and HIPAA Evaluate effectiveness of nursing interventions and adjust care plans as needed Submit accurate and timely patient notes/documentation Coordinate additional services as needed (LPN, HHA, Personal Care Attendant) Arrange referrals for services like PT or medical social work Supervise and support

LPNs, HHAs, and Personal Care Attendants Qualifications:
