Job Description

We are seeking a reliable and compassionate Registered Nurse (RN) in Taunton, MA to provide part-time in-home check-in visits for patients in the community. This role involves traveling to multiple residences to assess patient well-being, monitor health conditions, and ensure care plans are being followed. Haitian-Creole language skills are highly preferred.
Travel:
MUST be able to travel from the Cape Cod to Boston areas. Compensated for travel expenses
Schedule:
Part-time, flexible hours Travel required between patient homes
Pay:
$42.00 per hour
Responsibilities:
Conduct routine in-home visits to assess patients' overall health and safety Monitor vital signs and report any changes or concerns to the care team Ensure patients are following prescribed care plans and medications Provide nursing care and support as needed Maintain accurate and timely documentation of each visit Communicate effectively with patients, families, and healthcare providers Identify potential risks and report any safety or health concerns
Qualifications:
Active
RN OR LPN
