Position:
Assessment Nurse (RN)
Key Responsibilities:
Provide oversight, supervision, and support to field staff including CNAs, CHHAs, and other caregivers. Conduct patient assessments and develop individualized care plans in accordance with professional nursing standards. Collaborate with physicians and multidisciplinary team members to optimize patient care and outcomes. Educate patients and families on health maintenance, disease prevention, and care expectations. Ensure accurate documentation, compliance with regulatory requirements, and adherence to Joint Commission standards. Monitor clinical performance and provide coaching, training, and mentorship to staff. Act as a key liaison between patients, families, staff, and the BrightStar Care leadership team. Support case management, scheduling, and follow-up to ensure continuity of care.
Requirements:
Active RN license in the state of Massachusetts . Minimum 2 years of nursing experience (home health or acute care experience strongly preferred). Prior leadership, supervisory, or case management experience is a plus. Strong organizational, interpersonal, and communication skills. Passion for delivering high-quality, patient-centered care.
