We are seeking a highly motivated and detail-oriented MDS Coordinator to join our team and ensure accurate and timely resident assessments, compliance with CMS regulations, and optimal reimbursement. Responsibilities:
Complete and oversee accurate Minimum Data Set (MDS) assessments for all residents in compliance with federal, state, and facility regulations. Collaborate with nursing staff, medical directors, and interdisciplinary teams to develop, implement, and update individualized care plans. Ensure proper documentation for quality measures, resident care, and reimbursement purposes. Review and analyze clinical data to identify trends, support resident outcomes, and optimize facility reimbursement. Educate and mentor nursing staff on MDS processes, documentation standards, and regulatory compliance.
Qualifications:
Current RN or LPN license in Massachusetts. Minimum of 2-3 years experience in long-term care, preferably as an MDS Coordinator or in a related clinical role. Strong knowledge of
MDS, RAI, CMS
regulations, and reimbursement processes, with excellent analytical and communication skills.
