Job Overview RN Admission Nurse Home Health IsentCare is on demand/staffing agency. We are currently assisting sone of our top home care agency partners with a need for an experienced AdmissionsRN for Home Care to cover Woburn, Wakefield. Medford and the surrounding area. Flexible hours Weekends/Weekdays Competitive pay & mileage reimbursement Requires a minimum of 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency. Duties Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams. Administer medications and treatments as prescribed, while monitoring patients for any adverse reactions or changes in condition. Provide education to patients and families regarding health management, disease prevention, and treatment options. Maintain accurate and timely documentation of patient care activities and progress notes. Coordinate with other healthcare professionals to ensure continuity of care for patients. Assist with daily living activities as needed, including personal hygiene, mobility assistance, and meal preparation.
