Job Summary Provides skilled family-centered nursing care to clients in the home; completes client assessments, develops, and implements Individual Plan of Care, evaluates same, functions within the community as liaison between the Agency, its clients, other providers, and the people of the community. RN acts in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing and policies of Pernet Family Health Service. Essential Responsibilities
- Conducts home visits to provide skilled nursing services, completes client assessments, generates individual plans of care for each client, implements and evaluates same.
o Developing and initiating plan of care in collaboration with patient/caregiver/primary health professional. o Regularly re-evaluate patient/family/caregiver needs. o Conduct on-going evaluation and revision of care according to patient outcomes.
- Generates requests for provider orders and informs provider of client progress, changes in status, and discharge.
- Carries out orders and treatments utilizing appropriate nursing procedures and techniques.
- Documents pertinent findings and interventions in the EMR record in accordance with professional standards and legal requirements. o Documentation will be completed accurately and timely according to policies and procedures.
- Continuously develops knowledge of community resources and refers clients to appropriate resources.
- Responsibility for personal growth and development will be maintained through mandatory agency in-services, continuing education and education requirements for licensing.
- Serves as an advocate for client/families in obtaining needed services.
- Inform patient/family of rights and responsibilities. Treats every patient and family with dignity, trust and respect.
- Informing the physician of changes in the patient's needs and outcomes, updating the Plan of Care as needed
- Determine the scope and frequency of services needed based on acuity and patient/family needs
- Ongoing evaluation of the patient/family response to care and effectiveness of outcomes. Maintains communication with patients and family.
- Participates in quality activities which support the organization's mission, philosophy, customer service and process improvement goals.
- Precept new staff, nursing, and other students as applicable.
- Other duties as assigned. Minimum Requirements
- Graduation from an accredited School of Nursing; current licensure in the Commonwealth of Massachusetts.
- A minimum of two years of clinical nursing experience, one of which is in Maternal and Child health.
- Maternal Child Health and community nursing knowledge.
- Baccalaureate degree in nursing preferred.
- Valid driver's license and reliable transportation. Physical Demands
- While performing the duties of this job, the employee is regularly required to sit, stand, walk (up and down stairs), kneel, and reach with hands and arms. Must be able to regularly lift and/or move up to 10 pounds and occasionally up to 20 pounds. A nursing bag and infant scale must be brought to each visit and used during the visit. Work environment
- While performing the duties of this job, the employee frequently is exposed to outside weather conditions when traveling between sites/patient homes. Patient's home environments are variable with regard to heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. - This position offers an hourly rate starting at $42.
00 per hour . Actual starting pay will be based on relevant skills, qualifications, licensure, and experience.
