a2z Physical Therapy is seeking a skilled and compassionate Nurse Practitioner (NP) to provide in-home care with a focus on pain management and orthopedic conditions . This role involves delivering high-quality clinical care, managing chronic and acute pain, and coordinating comprehensive treatment plans tailored to each patient's orthopedic and rehabilitation needs. The ideal candidate excels in interdisciplinary collaboration, patient education, and independent clinical decision-making. We are currently hiring for PRN or part-time positions, with the potential to transition to full-time based on performance and our business growth. Key Responsibilities:
Perform in-home health assessments and focused exams, especially for musculoskeletal and orthopedic conditions. Develop and manage individualized treatment plans for patients dealing with chronic pain, post-operative recovery, or orthopedic injuries. Collaborate and communicate regularly with physical therapists to align goals and care plans. Coordinate referrals to specialists, radiology, and additional services as needed. Manage medications , including pain management prescriptions and controlled substances, with attention to safety and compliance. Provide patient and caregiver education on orthopedic care, pain management strategies, and medication adherence. Adjust treatment plans based on patient progress, outcomes, and evolving needs. Maintain accurate and timely documentation in the EMR system. Utilize telehealth technologies as needed for follow-ups and consultations.
Required Qualifications:
Licensed Nurse Practitioner (NP) in [State], with current prescriptive authority. 1-2+ years of experience in pain management, orthopedics, or related specialties. Experience in home health is preferred. Knowledge of orthopedic injuries, post-op recovery protocols, and chronic musculoskeletal pain. Certification in Pain Management (AANP/ANCC/ACNPs or equivalent) is a plus. Valid driver's license and reliable transportation.
