RN Case Manager, Discharge Planner

What you will do:
The Care Coordination ed Nurse (Case Manager) evaluates timeliness and appropriateness of patient care, and coordinates discharge or transition planning for a smooth progression to post-hospital care. Utilizing a collaborative process in conjunction with Care Management Coordinator, Social Workers and other health care team members, the Case manager will assess, plan, implement and evaluate the options and services required to meet an individual's health and health related needs. The Care Manager serves as an advocate for the patient, family, and organization to promote quality, cost effective outcomes. Active participation in program development and process improvement efforts is an expectation of the role of the Care Manager.

Salary Range:
Minimum 41.99/hour - Maximum 65.08/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
