Duration:
6 months with possible contract extensions
Work Schedule:
    Shift:
    8:00 AM
    • 4:30 PM
    Weekly Hours Guarantee:
    40 hours
      On-Call/Weekend Requirement:
      1 weekend every 4 weeks
      Mileage Reimbursement:
      Provided between patient visits
      • Job Summary We are seeking an experienced ed Nurse (RN) to provide comprehensive home health care services across Kent and Queen Anne's counties. The RN will be responsible for managing patient care episodes, completing OASIS documentation, and ensuring high-quality, coordinated care for home health patients.
      • Key Responsibilities 1. Clinical Care & Case Management
      • Provide skilled nursing care in accordance with physician orders and patient care plans
      • Manage a caseload of home health patients, ensuring continuity and quality of care
      • Perform comprehensive patient assessments and monitor changes in condition 2. OASIS & Visit Types
      • Complete and submit OASIS documentation for:
      • Admissions
      • Recertifications
      • Resumption of Care (ROC)
      • Discharges
      • Conduct routine visits (RV) and ensure accurate clinical documentation 3. Care Coordination
      • Collaborate with physicians, therapists, and interdisciplinary team members
      • Communicate patient progress, concerns, and care plan updates effectively
      • Coordinate services to ensure optimal patient outcomes 4. Documentation & Compliance
      • Maintain accurate and timely documentation in compliance with regulatory and payer requirements
      • Ad to infection control protocols and agency policies
      • Ensure all clinical activities meet state and federal home health standards

      • Queen Anne's County, MD
      • Qualifications Required:
      • Active ed Nurse (RN) license
      • Home Health experience (mandatory)
      • Strong knowledge of OASIS documentation
      • Ability to work independently and manage multiple patients
      Preferred:
      • Experience with Homecare Homebase (HCHB)
      • Strong case management and organizational skills
      • Excellent communication and patient education abilities
      • Dress Code
