We are seeking a compassionate and dependable PRN Home Health RN/LPN to provide skilled nursing services to patients in their homes. The nurse will assess patient needs, administer treatments, monitor health status, educate patients and caregivers, and collaborate with the interdisciplinary care team to ensure high-quality patient-centered care. This position offers a flexible schedule based on patient needs and agency census.
Essential Responsibilities:
Conduct skilled nursing visits in patients' homes. Perform comprehensive patient assessments and monitor changes in condition. Administer medications, treatments, and therapies as ordered by physicians. Provide wound care, catheter care, and other skilled nursing procedures. Educate patients and family members regarding disease management, medications, and safety. Develop, implement, and update individualized care plans. Accurately document patient care and assessments in the electronic medical record. Communicate patient status changes to physicians and clinical supervisors. Collaborate with therapists, aides, physicians, and other healthcare professionals. Ensure compliance with agency policies, state regulations, and Medicare guidelines. Participate in on-call coverage as assigned.
Qualifications:
Current RN or LPN license in the State of Maryland. Current CPR/BLS certification. Minimum one year of nursing experience preferred; home health experience preferred. Valid driver's license, reliable transportation, and auto insurance. Strong assessment, communication, and documentation skills. Ability to work independently and manage time effectively.
Schedule:
PRN/Per Diem Flexible scheduling based on patient needs and staff availability Weekday, evening, weekend, and on-call opportunities available
Pay:
$60.00 - $75.00 per hour
