Job Overview Join our dynamic healthcare team as a Bilingual Home Health Professional, providing compassionate and comprehensive care to diverse patient populations in their homes. This role is ideal for energetic, detail-oriented professionals fluent in both Spanish and English, committed to improving quality of life through personalized care. You will collaborate with a multidisciplinary team to deliver exceptional services that promote health, independence, and well-being. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews in both Spanish and English. Develop and implement individualized care plans aligned with patient needs and goals. Administer medications, injections, IV infusions, and perform wound suturing as needed following aseptic techniques. Provide specialized services such as dialysis support, ostomy care, tube feeding management, and ventilator assistance. Educate patients and families on disease management, medication administration, health coaching, and safety protocols. Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner while ensuring compliance with HIPAA regulations. Coordinate with physicians and interdisciplinary teams for discharge planning, case management, and urgent triage when necessary. Qualifications Valid licensure as an RN (Registered Nurse), PT (Physical Therapist), MSW (Master of Social Work), or relevant healthcare professional with current state certification. Bilingual fluency in Spanish and English is essential for effective communication with diverse patient populations. Proven experience in home care settings or related fields such as long-term care, hospice & palliative medicine, or hospital medicine. Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in ICD coding (ICD-9/ICD-10) is a plus. Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and medication administration protocols. Experience working with individuals with disabilities or developmental challenges is highly desirable. Ability to perform physical tasks such as lifting Hoyer lifts or assisting with mobility aids safely. Strong critical thinking skills for triage situations and acute pain management; experience in emergency medicine or trauma centers is advantageous.
