  • Fri varying hours between 7am
  • 5pm, 80 hrs.
biweekly Join our dedicated team at Meritus Health as a Clinical Navigator and play a vital role in supporting patients throughout their cancer journey. As a central point of contact, the Clinical Navigator collaborates with a multidisciplinary team to ensure patients receive coordinated, high-quality care across all stages of treatment. The Clinical Navigator is responsible for managing and guiding the care of cancer patients from diagnosis through survivorship. This role focuses on removing barriers to care, improving access to clinical and supportive services, and aligning patient care with established clinical guidelines. The Navigator serves as an advocate, educator, and resource for patients and families, both within Meritus Health and in the broader community. Act as the single point of contact for patients identified for navigation services. Coordinate care across all delivery sites and specialties to ensure a seamless patient experience. Assist patients in accessing clinical care, support services, and community resources. Collaborate closely with oncology physicians, nurses, social workers, and other providers to promote evidence-based care pathways. Track patient progress and ensure timely interventions and follow-up. Act as the single point of contact for patients identified for navigation services. Coordinate care across all delivery sites and specialties to ensure a seamless patient experience. Assist patients in accessing clinical care, support services, and community resources. Education:
ASN or higher required. BSN or higher preferred. ASN or higher required. BSN or higher preferred.
Experience:
Minimum of 2 years of clinical nursing experience in oncology or a related cancer care specialty. ASN
  • 10 years of acute care experience BSN
  • 5 years of acute care experience Minimum of 2 years of clinical nursing experience in oncology or a related cancer care specialty.
Licensure/Certification:
Current Registered Nurse (RN) license in the state of Maryland, or compact/ multistate license with ability to practice in MD. Active BLS/CPR certification in accordance with Meritus Health policy. Oncology Certified Nurse (OCN) preferred at time of hire; required within two years of employment. OCN preferred or other certifications will be considered for alignment with CoC and NABPC standards.
