MDS Nurse RN Job Summary:
Mount Airy Nursing and Rehab, a leading provider of post-acute care, is seeking a highly skilled and compassionate MDS Nurse RN to join our team at our Mount Airy, MD location. Role:
As a MDS Nurse RN, you will play a vital role in our rehabilitation centers, working closely with our interdisciplinary team to develop and implement individualized care plans for our patients. Your expertise will enable you to assess patient needs, identify goals, and prioritize interventions to achieve optimal outcomes. Your exceptional communication skills will also be essential in collaborating with patients, families, and healthcare professionals to ensure seamless care transitions.
Responsibilities:
Conduct comprehensive assessments and develop accurate MDS data to drive care decisions Collaborate with the interdisciplinary team to establish and revise patient care plans Identify and prioritize patient goals, developing measurable objectives and interventions Coordinate with rehabilitation team members to ensure effective care delivery Maintain accurate and timely documentation of patient assessments and care plans Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction Demonstrate a commitment to patient-centered care, prioritizing dignity, respect, and empathy
Requirements:
Current RN licensure in the state of Maryland Minimum 0 years of experience in a post-acute care setting (training and orientation provided) Strong knowledge of MDS regulations and guidelines Excellent communication, interpersonal, and leadership skills Ability to work collaboratively as part of a dynamic team
