Contractor's Duties:

hereby agrees to perform the following services: (1) Executing Health Home Intake Assessments and Health Home Reassessments. The assessments will be done face-to-face. (2) Completing Care Plans for each assessment/reassessment.

Hours:

approximately 20-25 hours per week, with a flexible schedule due to the contractual nature.
