Job Description
Nation's Infusion at Home, LLC, a wholly-owned subsidiary of Lifebridge Health Partners, is seeking a full-time, exempt Nurse Liaison to provide specialized infusion therapies and nursing services to patients and caregivers in their home or in alternative settings. Additionally, this role builds and maintains relationships with hospital personnel while coordinating end-to-end patient referrals, completing all required documentation, and maintaining thorough knowledge of the company's products and services. This position requires routine travel throughout the Lifebridge health system, service area, and patient homes (in and around Baltimore). The Nurse Liaison functions in hospital inpatient units, outpatient clinics, physician offices, and community/home environments; with a goal of 60% liaison responsibilities and 40% home infusion nurse responsibilities. Work hours will typically be Monday - Friday, 8:30 AM - 5 PM.
Key Responsibilities:
Serve as the clinical liaison between hospitals, providers, and the home infusion team to coordinate safe patient transitions to home-based care. Collaborate with physicians, case managers, discharge planners, pharmacists, and nursing staff to facilitate timely referrals and therapy initiation. Perform home infusion start-of-care visits and administer a variety of specialty infusion therapies in home and clinical settings. Provide patient and caregiver education on infusion therapy, medication administration, vascular access care, infection prevention, and emergency procedures. Deliver infusion nursing care to pediatric, adult, and geriatric patients, including medically complex populations. Demonstrate advanced vascular access skills, including peripheral IV placement and management of PICCs, ports, midlines, and central venous catheters. Monitor patient response to therapy, communicate clinical updates to the interdisciplinary team, and maintain accurate electronic documentation. Support continuity of care through strong relationships with hospital partners and commitment to exceptional patient outcomes.Requirements :
Active Maryland RN license Ability to pass a Criminal Background, drug test, and motor vehicle record check. Minimum of 1 year of acute setting nursing experience. Experience in a specialty involving infusion therapy including patient assessment skills, venous access skills and emergency patient care. Previous home health experience preferred Pediatric infusion experience preferred. Current BLS certification Excellent interpersonal, communication, and organizational skills. Graduate of an accredited school of nursing. Demonstrated expertise in peripheral IV insertion. Experience with bedside start of care programs. Experience with electronic medical record systems and home infusion software platforms. ACLS and/or PALS certification preferred. Ability to work independently and use critical-thinking skills Able to manage multiple referrals and priorities simultaneouslyWork Environment:
