REGISTERED NURSE
(RN) -
IN-HOME CARE Location:
Queen Anne's County, MD Position Summary The Registered Nurse (RN) provides skilled nursing care and medical support services to clients in their homes. This role focuses on assessment, treatment, medication administration, care coordination, patient education, and ongoing monitoring to ensure safe, effective, and compassionate care. Key Responsibilities Assess and monitor clients' physical, mental, and emotional conditions and document changes in status Administer prescribed medications and treatments according to nursing standards and established protocols Perform skilled nursing procedures including wound care, catheter care, specimen collection, and vital sign monitoring Develop and implement individualized care plans in collaboration with physicians, healthcare providers, and family members Educate clients and families regarding medications, treatments, and disease management Coordinate referrals, appointments, and transitions of care Maintain accurate and timely documentation in electronic and written records Communicate effectively with clients, families, physicians, and interdisciplinary care teams Participate in quality improvement activities, training, and continuing education Maintain compliance with HIPAA, regulatory requirements, and organizational policies Minimum Qualifications Current and active Maryland Registered Nurse (RN) license Current CPR/BLS certification Minimum two (2) years of nursing experience in clinical or home health settings Ability to read, write, speak, and understand English Basic computer and electronic health record (EHR) skills Valid driver's license and reliable transportation Ability to work flexible schedules, including on-call and rotating shifts when required Required Documentation Current Maryland RN License CPR/BLS Certification Tuberculosis (TB) test completed within the last 12 months Criminal background clearance Drug screening HIPAA and abuse/neglect training certification Valid government-issued photo ID Preferred Qualifications Bachelor of Science in Nursing (BSN) Three (3) or more years of home health or skilled nursing experience Home Health Nursing or Gerontology certification Experience working with elderly or chronically ill populations Physical & Professional Requirements Ability to work independently in clients' homes Ability to lift and carry up to 50 pounds Ability to stand, walk, bend, and perform patient care activities throughout the workday Strong communication, documentation, and interpersonal skills Commitment to professionalism, confidentiality, client dignity, and quality care
Pay:
$40.00 - $47.00 per hour Application Question(s): Have you provided care in a home health, long-term care, rehabilitation, or community-based setting? Are you comfortable assisting clients with activities of daily living (ADLs), medication support, and basic nursing care in home settings?
