About the
Role:
- Following a collaborative team model, the Nurse Navigator identifies and manages high risk patient populations (Bariatrics), coordinates their care.
KEY RESPONSIBILITIES
- Provides clinical oversight, guidance, and support to medical assistants
- Coordinates and manages the complete perioperative care pathway for bariatric and minimally invasive surgery patients, from initial evaluation through postoperative follow-up.
- Serves as a clinical resource for patients, providers, and staff by triaging and managing patient concerns, symptoms, and urgent clinical issues.
- Collaborates closely with surgeons, advanced practice providers, dietitians, behavioral health, and ancillary departments to ensure seamless patient care.

- Assists with care coordination, referrals, insurance authorizations, preoperative clearances, and patient education.
- Monitors patient progress throughout the surgical pathway, manages patient portal communications, triages clinical questions, identifies barriers to care, and coordinates timely interventions to optimize patient outcomes.
- Supports quality improvement initiatives, regulatory compliance, and MBSAQIP accreditation standards.
- Serves as a backup to the Clinical Manager, assisting with daily operations, workflow management, staff support, and problem resolution.
REQUIREMENTS
Education:
Bachelor's degree preferred
Experience:
1-3 years of experience
Certifications/Licensure:
Current MD RN license or ability to obtain license, American Heart Association Basic Life Support CPR
