Job Overview We are seeking a compassionate and skilled Registered Nurse (RN) to join our team in Solomons, Maryland. The RN is responsible for providing high-quality patient care that requires specialized nursing skills and clinical judgment. This role involves conducting patient assessments, developing and implementing care plans, educating patients and families, coordinating services, and ensuring compliance with applicable regulations, standards of nursing practice, and company policies. Essential Responsibilities Complete initial and ongoing comprehensive patient assessments at appropriate intervals. Contribute to the development and implementation of individualized patient care plans. Collaborate with patients and families to establish mutually agreed-upon healthcare goals. Provide skilled nursing care in accordance with the established plan of care. Educate and counsel patients, families, and caregivers regarding healthcare needs and treatment plans. Initiate appropriate preventive, therapeutic, and rehabilitative nursing interventions. Maintain timely, accurate, and complete clinical documentation. Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members. Coordinate referrals to additional healthcare services as needed. Evaluate patient outcomes and modify care plans accordingly. Assign and coordinate services provided by home care aides. Supervise paraprofessional staff and ensure quality patient care delivery. Participate in ongoing education, in-service training, and professional development activities. Assist with patient discharge planning and transition of care. Adhere to accepted standards of nursing practice and regulatory requirements. Utilize sound clinical judgment to effectively manage patient care. Support organizational initiatives, programs, and service excellence goals. Contribute to business growth through quality patient care and positive community relationships. Perform additional duties as assigned within the scope of licensure. Maintain CPR competency. Handle Protected Health Information (PHI) in accordance with HIPAA and organizational policies. Qualifications Required Graduate of an accredited School of Nursing. Current, active Registered Nurse (RN) license in the State of Maryland. License must be in good standing. Current CPR certification. Strong clinical assessment, communication, and documentation skills. Ability to work independently and collaboratively with interdisciplinary healthcare teams. Preferred At least 2 years of home health care experience. Home health, community health, or related nursing experience preferred.
