RN - Post Acute Long-Term Care (LTC)

Weeks Shift:
Days (7:00 AM - 7:00 PM) Responsibilities Provide direct and indirect nursing care to residents in long-term care, dementia care, skilled nursing, and rehabilitation settings. Deliver compassionate, resident-centered care to individuals with Alzheimer's disease and other forms of dementia. Perform comprehensive nursing assessments and develop individualized care plans. Administer medications, IV therapies, and specialized treatments as prescribed. Provide wound care management and monitor resident progress. Support patients recovering from surgeries, strokes, fractures, joint replacements, and other acute conditions. Collaborate with physicians, therapists, and interdisciplinary team members to ensure quality patient outcomes. Educate residents and families regarding treatment plans and ongoing care needs. Maintain accurate and timely clinical documentation. Float between units as needed to support operational and patient care requirements. Requirements Active Maine RN License or Compact Multistate RN License. Current BLS Certification. Associate Degree in Nursing (ADN) or higher from an accredited nursing program. Minimum 2 years of RN experience required. Long-Term Care, Skilled Nursing, or Rehabilitation experience preferred. Strong clinical assessment, communication, and organizational skills. Ability to provide compassionate care to geriatric and rehabilitation populations. Preferred Qualifications Experience in dementia and memory care settings. Skilled Nursing Facility (SNF) experience. Experience with IV therapy and complex wound care.
