Job Description

Make a Difference as a Hospice RN Case Manager Hospice of Lenawee is hiring a compassionate Registered Nurse (RN) Case Manager to provide high-quality, patient-centered hospice care in patients' homes throughout our 35-mile service area which includes Lenawee County and surrounding areas. Medical, dental, and vision insurance Life insurance + short/long-term disability 403(b) with employer match Supportive, mission-driven team environment Flexible, independent daily schedule Schedule Full-time (40 hours/week) Day shift Limited weekend rotation Holiday rotation (3-4 per year) RN Case Manager Responsibilities Provide skilled nursing care to hospice patients in their homes Manage a caseload of patients and families Develop and coordinate individualized care plans Monitor and manage pain and symptoms Educate and support patients and caregivers Collaborate with physicians and interdisciplinary team members Document care in accordance with regulatory standards
Qualifications Required:
Active Michigan RN license Graduate of an accredited nursing program CPR certification Valid driver's license and insured vehicle Ability to travel locally within Lenawee County and surrounding areas within our 35-mile service area
Preferred:
