13Weeks Position Overview:
The Clinical Care RN (CCRN) provides direct nursing care to participants in both clinic and home-based settings. This role requires strong clinical judgment, independent decision-making, and the ability to manage complex patient care situations involving acute and chronic medical conditions.Key Responsibilities:
Provide direct nursing care to geriatric and long-term care participants Triage emergent patient situations and apply advanced nursing knowledge independently Monitor and manage acute and chronic medical conditions in collaboration with PCPs Administer medications, treatments, and nursing procedures as prescribed Conduct home visits for wound care, lab draws, assessments, and follow-up care Utilize individualized care plans to deliver high-quality patient care Communicate effectively with caregivers, staff, and interdisciplinary teams Maintain accurate documentation using TruChart EMR Support patient safety, comfort, and continuity of careRequirements:
