At Faith Hospice, we utilize highly skilled nurses, medical social workers, chaplains, home health aides and volunteers to serve our patients, from medical and emotional care to pain management and bereavement counseling. Responsibilities Assess patient conditions and comfort, communicating changes to physicians, family, and caregivers Implement and revise individualized care plans, managing discomfort and providing symptom relief Administer medication, perform treatments, and coordinate with the Interdisciplinary Group for care planning Deliver specialized hospice training to staff, family, and caregivers for optimal patient care Educate patients and families on disease processes and provides emotional support Maintain communication with physicians and other disciplines Build trusting relationships and complete required documentation promptly Schedule 2nd shift | 2:30pm - 10:00pm Schedule repeats every 2 weeks: Week #1: Tues, Wed, Fri, Sat Week #2: Sun, Tue, Wed, Fri Eligible for pick-up pay, if you choose to pick up extra shifts an additional $4/hour if you pick up on a weekday an additional $6/hour if you pick up on a weekend an additional $8/hour if you pick up on a holiday Skills to be Successful Flexibility, resilience, and adaptability Self-directed and autonomous Strong prioritization skills Organized Comfort with navigating various technologies and computer applications Collaborative team player mindset Education and Experience RN licensure in Michigan At least 2 years of med/surg, acute care, or skilled nursing experience Hospice and/or hospital experience preferred Passion for hospice nursing Faith Hospice has a partnership with Christian Living Services and Holland Home.
