Clinical Navigator (RN/LPN) Skilled Nursing Admissions & Patient Experience Allegria Village | Dearborn, MI Help Patients and Families Navigate Their Next Chapter Allegria Village is seeking an experienced Clinical Navigator to join our Skilled Nursing team. This unique role combines clinical expertise, admissions coordination, patient advocacy, and healthcare relationship management to ensure every patient experiences a smooth and successful transition into our community. The ideal candidate will have at least three years of healthcare admissions, care coordination, discharge planning, case management, or healthcare marketing experience and enjoys building relationships with patients, families, hospitals, and healthcare professionals. As the Clinical Navigator, you will serve as a trusted resource for patients and families from the moment they are accepted for admission through their arrival and adjustment to the community. What You'll Do Patient & Family Navigation Visit accepted patients in hospitals prior to admission. Explain the rehabilitation process, facility services, daily routines, and admission expectations. Serve as a point of contact for patients and families throughout the admission process. Meet new admissions upon arrival and personally escort them to their room. Help orient residents to their room, the building, and available services. Contact family members within 24 hours of admission to provide updates and answer questions. Resolve patient and family concerns by collaborating with appropriate departments and ensuring timely follow-up. Advocate for exceptional patient and family experiences throughout the transition process. Admissions & Care Coordination Conduct clinical assessments and review medical records to determine admission eligibility. Coordinate with hospital case managers, social workers, discharge planners, and physicians. Assist with insurance verification and authorization processes. Collaborate closely with the Admissions and Marketing teams to facilitate smooth admissions. Help ensure room readiness and coordinate with departments prior to new admissions. Communicate admission arrivals to all departments to ensure proper introductions and support. Work with interdisciplinary teams to ensure patient needs are met from day one. Marketing & Relationship Development Build and maintain relationships with hospitals, physicians, rehabilitation centers, and community referral partners. Promote the Clinical Navigator program through hospital visits, phone outreach, email campaigns, networking, and social media. Participate in healthcare networking events and community outreach activities. Collaborate with Clinical Liaisons, Admissions Directors, and Marketing professionals to support census growth. Assist with marketing initiatives and referral development opportunities. Qualifications Active Michigan RN or LPN license preferred. Minimum three years of experience in healthcare admissions, healthcare marketing, case management, discharge planning, skilled nursing, or related healthcare settings. Strong understanding of Medicare, Medicaid, managed care, and healthcare admissions processes. Excellent communication, customer service, and relationship-building skills. Ability to work independently while managing multiple priorities. Knowledge of medical terminology and electronic medical records. Why Join Allegria Village?
