Job Description

Registered Nurse Case Manager (RNCM) Manageable caseloads, flexible 4-day work schedule, and meaningful hospice work that allows you to focus on quality patient care. The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory
  • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM. The schedule is four 10-hour shifts per week (4x10), meaning you work four days per week with three days off.

  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing care in accordance with hospice standards and physician orders
  • Coordinate care with physicians, caregivers, and interdisciplinary team members
  • Educate patients and families on symptom management, disease progression, and end-of-life care
  • Document assessments, interventions, and care plans accurately and timely in the
EMR HCHB

  • Participate in interdisciplinary team meetings and care planning
  • Support after-hours patient needs as part of an on-call rotation when applicable
