New Admission Visit:

$150 per visit Revisit /

Follow-up Visit:

$100 per visit Responsibilities Perform comprehensive patient assessments, including vital signs, health history, and ongoing condition monitoring. Develop and implement individualized care plans in collaboration with physicians and care teams. Administer medications, injections, and treatments as prescribed, following proper clinical protocols. Provide skilled nursing care such as wound care, IV therapy, catheter care, and patient education. Accurately document all patient interactions and care provided using electronic health record (EHR) systems. Monitor patient progress and promptly report any changes in condition to the appropriate healthcare providers. Educate patients and families on disease management, medications, and home care techniques. Ensure compliance with all state regulations, agency policies, and HIPAA standards. Requirements Active and valid Registered Nurse (RN) license in the state of Michigan . Minimum of 1 year of nursing experience; home care experience preferred. Strong clinical assessment and critical thinking skills. Proficiency in EHR/EMR systems for accurate documentation. Ability to work independently and manage time effectively in a home care setting.
