Month Service Territory:
Travel throughout Wayne, Oakland, and Macomb counties (approximately 40-60 miles per day ).
Patient Load :
Approximately 6 home visits per day
Orientation :
2-4 weeks (paid) Job Summary We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop and implement care plans, coordinate with interdisciplinary healthcare teams, and ensure high-quality patient-centered care. This position requires independent clinical judgment, strong communication skills, and the ability to travel throughout the assigned service area. Responsibilities
  • Provide direct nursing care to patients in their homes according to physician orders and care plans.
  • Assess patient conditions, monitor progress, and document clinical findings accurately.
  • Coordinate care with physicians, physical therapists, occupational therapists, speech therapists, social workers, and other healthcare professionals.
  • Educate patients and families regarding treatment plans, medications, and disease management.
  • Maintain timely and accurate clinical documentation in compliance with agency and regulatory requirements.
  • Manage an average caseload of approximately 6 patient visits per day.
  • Travel throughout Wayne, Oakland, and Macomb counties (approximately 40-60 miles per day).
  • Participate in orientation and field training with a preceptor. Requirements
  • Minimum 1 year of Home Health RN experience.
  • Graduate of an accredited Registered Nursing (RN) program.
  • Current Michigan RN license.
  • Current BLS certification through the American Heart Association (AHA).
  • Valid driver's license with a satisfactory driving record.
  • Reliable transportation and telephone access.
