Job Description

Facilitate and assist new and current team members meet the essential functions of the Support Coordinator Social Workers role, understand DAAA practices, policies, and procedures while meeting practice objectives related to organizational pillars and goals. Maintain monthly competency rates of 95% or better of assigned caseloads. Serve as a role model, provide leadership, and demonstrate professionalism. Serve as a resource by guiding new and current team members to appropriate resource materials and management when appropriate. Guide and assist new team members in the integration into DAAA culture and environment. Nurture, support and guide new and current team members through training and mentoring. Monitor time management and productivity of new and current team members as directed by Manager; provide supporting documentation. Provide valuable formative and summative feedback from the training and mentoring process to assigned Manager weekly or as indicated. Perform all other duties as assigned. Emphasize the importance of collaborating with peers (Registered Nurses) in coordinating services. Abides by
DAAA's and Wavier Codes of Ethics Education:
Registered Nurse Competency Maintain 95% compliance rating of all assigned core services.
Experience:
Two-three years of experience completing intakes, pre-screening, and/or assessments, preferably in a health care or human service setting serving the elderly and/or disabled population. Knowledge of Medicaid and Medicare regulations. Ability to complete assessments using computerized screening tools. Excellent Customer Service skills and ability to de-escalate callers. The Social Worker Preceptor (SWP) will teach, support, counsel, coach, evaluate and serve as a role model for newly hired Social Workers. The SWP is expected to create an environment conducive to learning with an inviting welcoming spirit. Each new Social Worker will be assigned a SWP that will provide an opportunity for new staff shadowing, exposing them to every aspect of their new role as a Social Worker Support Coordinator. The SWP will provide on-the-job training including but not limited to performing in-home face-to-face participant enrollments, reenrollments, and properly addressing any participant conditional changes from pyscho-social perspective. The SWP will be asked to train and support existing Team Members as assigned by the Managerial staff when/if applicable as well. The successful candidate must be a servant leader, demonstrating excellent interpersonal skills with experience motivating, and developing high-performing individuals. Must be able to manage a caseload of 50-70 participants and demonstrate strong analytical and problem-solving skills, exercise sound judgment and ability to read/ analyze complex documents while exhibiting critical thinking skills. The ideal candidate must be forward-thinking, with high ethical standards and display an appropriate professional image. The successful candidate must have outstanding communication skills, commitment to excellence and innovation, a passion for Detroit Area Agency on Aging (DAAA)'s mission & vision. The candidate must clearly articulate DAAA's Organizational Pillars and Strategic Goals to new employees. To accomplish this job successfully, the selected candidate must be able to maintain stated competency levels of designated caseloads. This position is expected to submit a weekly progress report to their assigned Manager until the new employee successfully becomes independent. The progress report must encompass observed/assigned duties, work performances, strengths/weakness, and any other job-related comments/concerns.
