  • Contract Job Summary We are seeking a dedicated and compassionate Registered Nurse specializing in Long Term Care and Rehabilitation to join our healthcare team.
This vital role involves providing comprehensive nursing care to residents in long-term care facilities, rehabilitation centers, assisted living communities, and post-acute care settings. The ideal candidate will utilize their extensive clinical expertise to promote patient well-being, ensure safety, and support recovery and quality of life for diverse patient populations. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical and rehabilitative support. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and review of medical histories to develop individualized care plans. Administer medications and treatments accurately, including injections, IV infusions, wound suturing, and specialized therapies such as botulinum toxin treatments. Monitor patients' responses to treatments, manage acute pain, and adjust care plans accordingly to optimize outcomes. Collaborate with interdisciplinary teams to coordinate discharge planning, therapy services, and ongoing care needs. Maintain detailed and compliant medical documentation using electronic health record (EHR) systems such as Epic or Cerner, ensuring adherence to HIPAA regulations. Provide education and support to patients and their families regarding health management, medication administration, and post-discharge instructions. Assist with complex procedures including catheterizations, tube feedings, ostomy care, and respiratory support such as ventilator management. Participate in infection control protocols and ensure aseptic techniques are rigorously followed across all patient interactions. Engage in triage activities for urgent cases within long-term care or rehabilitation settings to prioritize patient needs effectively. Support residents with cognitive impairments such as dementia or Alzheimer's disease through memory care initiatives and behavioral health strategies.
MUST HAVE
2 years of RN
  • LTC experience
  • REQUIRED BLS
  • AHA RN
MI License Shift Timings:
12 hrs day shift and 36 hrs weekly HC1
Pay:
$50.00 per hour
