• Part-time, Full-time Rite Choice Home Health Care is looking for Contingent RN's to join its growing team!

Our Main Office is located in Canton, MI, but we are looking for Case Managers to cover East Detroit. Rite Choice Home Health Care is dedicated to providing the highest quality home health care services throughout Southeast Michigan. We strive to meet the needs of each patient, physician, case manager, and other community professionals. Whether our homebound clients are in need of a highly Skilled Nurse, Physical Therapist, Occupational Therapist, Speech Therapist, Social Work or a Nursing Aide we can help. The Registered Nurse must be experienced in home care and able to deliver high quality in-home patient care to our patients. We are only interviewing and considering serious candidates for our team. Pay scale is per diem/ per visit. $110 for SOC/ROC/Recerts, and $80 for RN ongoing follow up visits, based on experience and coverage area. Qualifications for this position include (but are not limited to):

• Have excellent and positive communication skills with exemplary customer service.
• Must understand the team approach and be a supportive team member.
• Must be able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, assisting with exercises, etc.
• Assumes responsibility for an assigned caseload of patients to a developed plan of care. Conducts OASIS assessments at appropriate time points.
• Makes planned home visits to the patient for assessment, instruction, supervision, and provision of nursing treatments according to the Plan of Care.
• Coordinates therapists, home health aides, and others who provide Services to each patient to form an interdisciplinary team to ensure consistent and appropriate care.
• Infusion therapy, including removal of PICC lines (in home), . Lab draw experience and proficiency a must
• Complicated wound care and wound vac experience.
• (Trach) Tracheostomy Care, maintenance.
• (Cath) Foley and Suprapubic catheter care and maintenance. Strong documentation both in content and timeliness Requirements of this position include:
• Graduate of an accredited school of nursing. BA degree preferred for Registered Nurse but experience is also preferred.
• Experience Home Health Care with strong grasp of home health.
• Must have at least two-year clinical experience
• Kinnser/Oasis training preferred.
• Home care experience required.
• Current professional licensure active in the State of Michigan.
• CPR certification required.
• Current Michigan driver's license and/or reliable transportation with proof of automobile insurance of personal vehicle for business use
