This role involves delivering personalized, high-quality care in the comfort of patients' homes, ensuring their health and well-being with professionalism and empathy. Responsibilities Provide skilled nursing care to clients in their homes according to individual care plans. Monitor patient health status and report changes to healthcare providers and family members. Administer medications and treatments as prescribed. Educate clients and their families on health conditions, medication management, and overall wellness. Maintain accurate and timely documentation of patient care and progress. Collaborate with healthcare teams to coordinate comprehensive patient care. Ensure compliance with all health and safety regulations and company policies. Requirements Current Registered Nurse (RN) license in the state of Michigan. Minimum of 1-2 years of nursing experience, preferably in home health. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid driver's license and reliable transportation. CPR and Basic Life Support (BLS) certification preferred.
