Our team uses Home Health Notify , a HIPAA-compliant communication platform that keeps you instantly connected with your team, patients, and updates in real time reducing delays, confusion, and unnecessary back-and-forth. We are seeking a dedicated RN Case Manager who combines strong clinical judgment with empathy, efficiency, and the ability to guide patients and families through complex and emotional journeys. This role primarily serves Oakland, Macomb, Wayne and Genesee counties. What You'll Do Complete comprehensive hospice assessments and develop individualized plans of care Provide skilled nursing focused on comfort, symptom management, and quality of life Educate patients and families on disease progression, medications, and expectations Coordinate care with the interdisciplinary team (physicians, social workers, chaplains, aides) Use Home Health Notify (HIPAA-compliant app) for real-time communication, updates, and care coordination Utilize Careficient EMR to streamline documentation and care planning Ensure documentation is timely, accurate, and compliant with hospice regulations Participate in SOC, recertifications, and ongoing patient evaluations Support patients and families emotionally throughout the hospice journey What We're Looking For Graduate of an accredited school of nursing Active Michigan RN license Minimum of 2 years of nursing experience ( hospice or home health preferred ) Comfortable using technology for communication and documentation Strong clinical judgment and excellent communication skills Compassionate, calm, and patient-focused Valid driver's license, auto insurance, and reliable transportation currently we cannot offer a full caseload, we are looking at around 10-12 visit per week, our pay model is per diem and per visit while we grow our census.
