We're currently hiring a Full-Time RN Case Manager to provide skilled nursing care to patients in their homes. This position follows a per-visit compensation model , with a guaranteed weekly minimum pay —giving you the freedom and earning potential of a visit-based role, backed by the consistency of a guaranteed income. A company vehicle and fuel card are provided to support travel throughout our service area.

What You'll Do:

Serve as the primary nurse for a manageable caseload of home health patients Complete skilled nursing visits, including Start of Care (SOC), Recertifications, Resumptions, and routine follow-ups Conduct comprehensive assessments and develop individualized plans of care Provide hands-on skilled nursing care such as wound care, IV management, and medication administration Educate patients and caregivers on health management and care plans Supervise LPNs and Home Health Aides as needed Accurately document care in our user-friendly EMR Collaborate with the interdisciplinary team to ensure coordinated, high-quality care

Professional Development Qualifications:

Active RN license in the state of Michigan 1+ year of nursing experience (home health experience preferred) Proficient in clinical assessment and documentation Experience with OASIS and home health workflows is a plus (training available) Compassionate, dependable, and able to work independently in the field Valid driver's license and a clean driving record
